Everything besides the photo is optional — it helps the office code the receipt. With several photos attached, each one is coded on its own: the button sends the first, then the form comes back for the next. Anything with "Keep for next" ticked stays filled in; the rest clears.

Put this on your home screeni

Android (Chrome): tap the ⋮ menu, then "Install and create shortcut" (older Chrome calls it "Add to Home screen"), then Add.
Samsung Internet: tap the ☰ menu at the bottom, then "Add page to" → "Home screen".
iPhone: tap the Share button ⬆ in Safari, then "Add to Home Screen".
